Coding Need Help

Is there anyone who encountered problem on billing or coding BCBS using a UB-04 Form?? I just want to know what is the right occurrence span code to used on box 32 of the form?? and i'm also asking if anyone knows about the denial codes 018 4. it is referring to a modifier but not sure what it is... I am billing on a hospice, but my claim is always rejected due to a occurrence span code is not illegible, which i used 11 and then date of the service.. and i am not sure what box on the form is the denial code 018 4. which it says invalid modifier... BCBS not telling you any information about the right code to used, so hope any one can help me here or just give me a hint... thanks... :D

Since it's a very carrier specific issue it is hard to assist.  What state BCBS is it?  If you are billing for hospice services I would think the occurrence span would be the date the hospice started to the final date on the bill (services you are billing for on this claim).  First, is the occurrence span required by BCBS?  It is a conditional field.  If it's not required, maybe eliminating it would solve your problem.  As for the 018 4, that is carrier specific and I have no idea what it means.

BCBS Missiouri, and yes they required that occurrence span code.. they have denial codes that i didn't get it, especially it is regarding about modifier.. most of them are 018 4. 018 1. 018 07. , the last time the denial code 018 07 is in the box 44 of the ub-04 form, but the rest i don't know what box it is on the form,  if just anyone can tell me i'm sure i can figure out the problem.. thanks...

I don't have any experience with BCBS of Missiouri.  Since we only have the codes which are specific to BCBS Missouri I'm afraid I can't be of much help.  I would try contacting them to see if they have a provider rep who may be able to assist you with understanding their eobs & billing requirements.
