Can you please tell me where the best place is to order claim forms?  I've been purchasing them as I need them at a local office supply store.

I get mine from AmeriFile (amerifile.net) their file folders and stickers are cheap too!

There are a ton of places to buy them.  I believe we get ours from Medical Arts Press, but I don't do the ordering.  If you google it you can compare prices.

Medical Arts Press is a good place to get your Claim Forms, just my opinion.
