Medicare non-covered mental health services

What form should a Medicare Beneficary complete if they are seeing a non-Medicare provider? For example the Beneficary sees a LPC and the service can not be billed "Incident To".

You have them fill out an ABN before the service, so they can decide whether or not they want the service.

ABN - Advance Beneficiary Notice - An Advance Beneficiary Notice (ABN), also known as a waiver of liability, is a notice a provider should give you before you receive a service if, based on Medicare coverage rules, your provider has reason to believe Medicare will not pay for the service. You may receive an ABN if you have Original Medicare, but not if you have a Medicare Advantage Plan. The ABN may look different, depending on the type of provider who gives it to you.
