Hello-

These are all good questions. I've only been in the billing bus. for 18mos. but here are my answers:

1. No, I do not charge a set up fee, however my contract with my clients states they give me 3 business days to learn their existing software (assuming they have any), and if necessary, to input provider and patient data into my own system.

2. Yes, I charge a monthly fee. It is a flat fee (the same every month) This fee is based upon many factors, such as # of claims each month, if they want me to post payments to their ledger, if they want me to send invoices to their cash customers w/ outstanding balances etc.

3. I always try to submit claims through the doctors own existing software. If they have none, I use Office Ally.

-Best of Luck  :)

I don't have a billing service, but I work for several, and here is my opinion:

1. If you are transitioning the client to your software, charge a set-up fee. If you are using their software(this is what is preferable), then no set-up fee is necessary.

2. I would charge a monthly fee, not a percentage.

3. Of course, you input their claims regardless of what software you use. Again, it is preferable to use their existing software.

ALWAYS.. and how much depends on the client and work they want done. My setup fee is not just fee to cover my expenses in starting up an account but it's also a "security" measure as I can always tell a good client from a bad client by their reaction to my setup fee.

I charge flat monthly fee on sliding scale to accommodate practice growth.. I get paid for ALL of my work.I do access my clients PM system, but I'm entering all of it.. from start to finish.. I have my own login/access.
