I have a resident billing a 99204 and then he is seen by a physician who bills a 99214. Should it be coded as 99204 25 & 99214?

should bill 99214 mod 25 if seen pt for a different unrelated dx.

No it is the same DX. DX of myelomeningocele 741.02, it is the most severe case of spina bifida

Are these doctors in the same practice? I don't believe you can charge for the two E/M's for same dx per day.

I did find the resident using the verbage "counseling" in doucmentation. Since Medicare doesn not accept consultations we have a cross walk of codes to referr to . Eg. 99241 should be billed as 99204. That is what I think happened. How do I handle that I am not sure, I put Modifier 27 on to the 99214 (that is the second visit of the day and the 1st visit is documented recheck, so this is an established patient.
