If we have 7 or more lines of service for one patient encounter, how does a hard copy CMS 1500 look like according to Medicare? Should a "Page 1 of 2" and "Page 2 of 2" exist, or would it be a whole seperate bill? If it is the latter, than there should be some sort of Bill ICN listed correct? Also, how would this same scenario apply if billing the CMS-1500 electronically(837)?

When billing electronically it goes thru as one claim.  When printing, it will print on 2 separate forms.  The ICN is assigned by Medicare when the claim is processed and "page 1" or the first 6 charges will have 1 ICN and the 7th charge will have a different ICN.  You don't need to mark the forms prior to mailing them.
