D.O. just started using these codes, not sure how to go about it.

99215 mod 25,  722.10, v82.89, 721.3
98927 mod 59, 722.10, 721.3, 720.2, 739.1
97140 mod gp kx    722.10, 721.3, 720.2, 739.1
97110 gp kx     722.10, 721.3, 720.2, 739.1
97530 gp kx 722.10, 721.3, 720.2, 739.1
Medicare is bundling the 97140 97110 97530 into 98927. Provider and massage therapist spent 1.5 hrs with pt.

Without being able to see the patient's chart or speak to the provider it's impossible to know if another modifier would be apporpriate, however, Medicare does bundle those services together unless there is a modifier indicating that they are distinctly separate services.  Also, if I'm not mistaken, Medicare does not reimburse for the osteopathic manipulation and the PT services on the same date.
