After Lytec 2008 crashed we are receiving error message from clearinghouse stating that referring physician secondary qualifier missing or invalid. We have tried deleting several fields but the same message appears. Has anyone seen this rejection code before??

Did you contact the clearing house?  Ask them which segment and loop is affected and what info is in there, what info is supposed to be in there.  Then contact your vendor and tell them what the problem is.
