Love your site!  Can someone tell me which form we would need to use to bill home health care?

   Glad you find the site helpful.  Home Health Care billing is done mostly on UB04 forms.

Use the standard CMS 1500 standard insurance claim form dated 08/05
You can order them online at Amazon etc..

Home Health billing is sometimes done on the CMS 1500 and sometimes on the UB04, depending on the services you are billing and how you are credentialed with the insurance carriers.  Some require the UB04, some want it on the CMS form.
