FACILITY BILLING QUESTION

In billing for PHP or IOP I use the UB-04 and revenue codes 912 or 905.  Are there CPT codes for these as well?  I am being told that I need to bill using CPT codes.  In locator box 43 on the UB-04 I use a  description of the service.

Is this is for group therapy other then multi family?  If so then 90853 would be appropriate

We have used the 90806 and the 90853 in conjunction with these rev codes in the past.

CPT Codes are only for Physician Based billing.  If you are billing on a UB-04, Revenue codes should work.

Some insurance carriers require both rev & cpt codes for outpatient.

Agreed Michelle, In our facilities we have to bill with both Rev and CPT codes to some on our carriers on the UB-04
