Michele,

Hi, I need help & don't know where to turn. We have Carefirst BCBS of VA. My husband went to a substance abuse facility, last summer and we have to file our insurance claim ourselves. I am having a lot of trouble filing. I submitted the bill I was given from the rehab facility along with BCBS's form and got a letter back from BCBS saying they could not process the claim because there were no cpt codes on the bill. I called the rehab facility and they sent me a list of what their codes translated to but they gave me 3 digit revenue codes as opposed to a 5 digit cpt codes (ex. their translation states that "Treatment/Partial Hospitalization" code is 912). I called back BCBS & they told me I needed the 5 digit cpt codes but would not provide me with what those codes are. I have put in several more calls to the rehab facility but they have not returned my calls.  I am at a loss of what to do. We had to borrow money from relatives in order for my husband to get the help he desperately needed and now it looks like we will never be able to pay them back any of it. Do you have any ideas on how I can get this claim filed?
