What goes in Field 51 on a UB04 form?

Question
We are still having some problems figuring out what information goes in the UB04 fields. In particular field 51 - Health Plan Identification Number. The Uniform Billing Editor states that this field should contain the number used by the health plan to identify itself. Where do I find out this information?

Hi Cindy,
Field 51 is for the number assigned to the provider by the payer indicated in field 50. It may also be referred to as a legacy number or provider number. If the particular insurance company assigns a provider number to you, then you would put it in field 51. If they do not assign any number to you, you may leave it blank. If you are not sure, you can look on an old eob from the insurance carrier and see if it has a provider number listed on the eob. Another way you can check is to call the insurance company and ask if they assign provider numbers, and if yes, what is yours. There should be a place in your practice management system to put the number so it will print on the form.
