ICD-9 for Mohs Defect

The patient has a repair of the Mohs Defect by adjacent soft tissue transfer. Since the excision was not performed, but there is the confirmation of a malignant neoplasm, would a Neoplasm diagnosis be used or a V code for history of? I recently found a post by another coder stating the V code should be used, but I thought that if the patient was still currently receiving treatment that the neoplasm code should be used, or would the Mohs defect repair not be considered treatment?  Thank you!

I am not a certified coder but I believe the neoplasm dx would be appropriate.  Just because the excision was not performed doesn't take away the fact that the patient has the neoplasm and that the services provided were for that dx.

Any certified coders want to weigh in?
