Sending claims before note is signed off...

Someone asked me if you can do this and I say no but can't find any documentation.  Anyone?

That is too vague to answer. Are you a billing company? Are you in an office? Either way if you are IN the office, yes the notes/chart / superbill should be signed off on by the physician.. if you are a billing company.. absolutely, positively you should receive a sign off on the daysheet/superbill/charge slip

And if you are talking about an Electronic Medical Record system that can send billing data to the billing portion of the software - I know that some programs will give the doctor an error message if he tries to send the data to billing before the note/chart is signed off on.  But the doctor can override that error message and send the data anyway.

We were asked to take on a client who had such a system.  We turned them down because there was no way in the software to absolutely block the doctor from sending data to billing before the note/chart was signed off.  We didn't want to be in the position of having sent the billing to the clearinghouse and later have the doctor change things in the chart so that it no longer supported the billing that we sent forward.

I don't blame you.. that audit trail is absolutely the be all and end all in liability. In any system I've worked in.. once the status of the claim was "filed" you could NOT change a dx or anything like that to protect the audit trail. If you needed to change a diagnosis you had to do a corrected claim which is how its supposed to be. I've seen many doctors complain that's a problem when really it's a problem when YOU CAN change/edit those fields after submission!
