Are you allowed to bill for more than one unit for 90834 / 90837 or 90847?  How do you bill if therapist sees the patient for 90 minutes?

The short answer is no.  Only one unit can be billed, and as these codes are "timed" codes, you would use the code that is closest to the amount of time spent with the patient (in this case it would be 90837 60+minutes).

If the patient was in some sort of crisis, and this necessitated the 90 minute session, then different CPT codes from those that you stated would apply.

In what circumstances with mental health billing can you use more than one unit?

Psychological testing can be billed with units.  I believe that is all.
