Medicare reactivation

Hi,  I am a newby and already loving this!  Okay, one of my clients had to reactivate his agreement with Medicare.  In doing so they assigned a new PTAN # to the Dr and the old PTAN # is not in the system any longer. Consequently, my clearinghouse needed the enrollment redone. SO we sent all the necessary documentation and forms to Medicare.  Question... Will the new PTAN # be associated with the Dr's NPI number once Medicare completes this reactivation?  When the claim is done, is Block 32a and 33a given the facility NPI number? Block 24 is the rendering Doctor's NPI #?  Thanks in advance for your help.

Box 24J is the rendering drs individual NPI.  Box 33A is the billing NPI so if the dr uses a group NPI & EIN, the group NPI goes here.  Box 32a is the NPI for the place that the services were rendered so if they were done in the drs office, then the group NPI would be in 32a as well.  If there is not group NPI then then individual NPI goes in 33a & 32a, but 24J would be blank.
