when putting more than one sevice date on a claim, is there a time frame for doing this?

second question,
If a provider has old claims to collect on is there a time frame for that also?

You can put multiple dates of service on a claim, the only limit I have found is that many insurance carriers do not like multiple years on the same form. 

As far as time frame, it varies by insurance carrier.  Some have 90 days, some allow 1 yr, etc.
