Dead/No Load Miles question for NMT / NEMT

Hello Everyone,

I have a basic question regarding billing for Dead / No Load Miles. We are considering ABC as an organization name here for example

i. If a driver is based somewhere other than the location of NEMT office, is ABC allowed to bill for dead/no load miles for authorized trips for the distance from the office to the patient and back, or just for the actual distance the driver travels from the driver's physical starting point to the patient's location and back?

ii.  Does the answer to (i). change if Medi-cal originally authorized ABC to perform a trip in Sacramento using a driver who started at NEMT' offices in Los Angeles, but ABC later found a driver who was located in Sacramento to make subsequent trips, or should ABC only bill for dead/no load miles for those subsequent trips from the driver's actual starting point?
