: 64472-76 used more then once

I have encounterd a problem with submitting 64472-76 when the Dr. does a couple of locations.....they reject(Medicare)....... I have read in a research note that they (the codes) can only be submitted once...Has anyone else had this problem and is there a work around?

Are you billing the 64472 on multiple lines?   You should be billing the 64472 just on one line with the number of levels indicated with units.  I wouldn't use the 76 modifier since the description of 64472 is 'each additional level'.  They already know other procedures have been performed.

Hi Jonah,

I looked the procedure code up and noticed it is an "Add" on code. As Michele stated it does need to be filed on it's on line, following the primary CPT code.  We  can not use modifiers with add on codes, Also medicare will only pay certain dx codes for this procedure, What dx code(s) are you billing with this procedure?
