We are a physical therapy office and we normally bill as a group. Cigna credentials/contracts the individual instead.  I read through the How to Complete the HCFA 1500 line by line and I want to confirm what I read.
In box 32- this is the location that the patient was seen at and 32A would be that locations NPI.  Box 33 would be the individual provider- name and address and 33A would be his individual NPI number (the same number that appears in 24J)

Even though Cigna credentials/contracts the individual you can still bill with the group information.  You are correct on the Box 32 info.  I would put the group info in box 33 including the group NPI.  I would put the individual NPI in 24J.

I just don't want you to be behind, it's not HCFA 1500 and hasn't been in a while, it's the CMS 1500 and there are some differences.
