CPT 99406 Medicare + Modifer

I would like to know what modifier is used when billing CPT 99406 (smoking cessation) to Medicare. I used 25 but the claim was denied with RA message "PROCEDURE CODE IS INCONSISTENT WITH MODIFIER USED OR A REQUIRED MODIFIER IS MISSING".

Also note that on the same day we bill 90837 modifier (59) and 90853. Please help!

Modifier 25 cannot be appended with 99406 so remove the modifier and resubmit the claim.
