What is a reasonable fee per claim to charge for psychotherapy?

I have never billed for them, I base fees on the claim volume, claim amount, my time, difficulty of service,

It all depends on your experience, location, type of claims, volume, pricing of clearinghouse you are going with. This is why I rather like hourly or flat rate because you can begin your formula based on what you want to make and what your worth. Also going on a per claim rate means you might have to have a menu of services. It's best to conduct a full office profile to determine a fair price per client.
