GETTING THE FIRST CLIENT

I am new to this biz and I have been making calls to Physical Therapists for the past one month and I have had a few call backs and one that is desperately interested. Others have told me that they either bill in house or have billing companies that they have done biz with for a long time. The one desperate PT needed a new billing company as at “yesterday” but has the following concerns:
-She knows that Medicare has a cap on 24 visits/$1800 per patient but wants to know if I knew the exceptional/special diagnostic codes that would enable her bill beyond the 24 visits so that she could make more money. She said she does not bill Medicaid and Empire Blue (NY).
-She has stopped billing because the present billing company is not good enough and wants a new one in NY State.
- She wants an agreement as early as 8/10/10 because she wants to resume billing again.
-She wants to pay 5% on collected claims. She bills about $100,000/month but collects only about $11,000.00. To me this is too low.
-She wants references even though I am new.
I know it has been boring reading this but I am in desperate need of advice because I need to start working in this business. I would not also mind reading references for information.

It is illegal for medical providers to enter into a percentage based contract in the state of NY.

As far as references, be honest but at the same time give them some personal or other professional references, nothing wrong with that because the provider may want to check things like dependability, trustworthiness, etc.
As far as her wanting the diagnosis to get "around" the Medicare cap.. that's a no no, she should code as per her documentation on a patient by patient basis. Billing and changing codes for the sake of reimbursement, is a sure fire way to end up in hot water, same for you since it would land you in trouble.

Some things I would do is find out exactly what problems she's having with the present billing company and focus on those areas where you can make a difference in her practice.

I thank both of you for your contributions. I would call her back tomorrow afternoon since that is the time she is free to answer calls and discuss with her again.

However, I am also open for any additional advice from both of you or anybody about this and how to get a first contract.
