Billing CHDP

I am in California, and currently billing for a pediatrician. She will be accepting CHDP soon. I know the basics like the visit info goes on the PM-160 form, and the billing codes. But I don't know, once the form is forwarded to me from the doctor is it complete and ready to be sent out? and if so where do I send it?  Also, what is the claims filing limit for these?  And is there anything special I should know?Anyone that can answer, Thank u for your help.
