Hello,

I would like to know if anyone knows the guidelines for coding nonunion fractures. The patient had a metatarsal fracture in January, he returns in March with a Jones fracture, fifth metatarsal nonunion x2 months. The physician repairs the fracture with a plate and screws. My question is, would this be coded as a fracture repair (28485) or a nonunion repair (28322). Thank You  ???

Are you a coder?  I am a biller and I wouldn't want to answer this. I would want the provider or coder to let me know.  You do give details but without having the patient's chart I'm not sure anyone can determine the correct coding.  Based on the information below I would think it would be the latter but again, I'm not comfortable making that decision in my position.  I would have gone back to the provider to get the appropriate code.

Medicare guildelines follow that a fx is considered a non-union when it hits 90 days.
