: CMS-1500/UB04 sample with ICD-10

Hi! I'm Edward Ortiz. I work at an outsourcing company here in the Philippines that does medical billing for some US clients.

I recently read Ms. Alice Scott's article about ICD-10 and made my own presentation about ICD-10 for the management. As early as now, my company is already preparing for the ICD-10 transition that's going to happen in 2013. What we need right now is a sample of what CMS/UB would look like using ICD-10. I know ICD-9 is still being used in the US but someone may have a sample coming from another country, like Canada or France. I've been trying to search for a sample for more than a week now but to no avail.

If anyone can give us a CMS/UB sample(of course, with claimant detail being deleted), it would be a great help. Thank you so much.

The current claim forms will not work with ICD-10.  They will have to be adapted to accompany the longer numbers that ICD-10-CM contains.  if you want to use the current CMS 1500/UB-04, you can find blanks on the internet and fill out block 24 yourself.

The claim form is going to be the last part of the worries. There are bigger fish to fry like making sure PM softwares are updated to accommodate the fields. You can bet that there will be a new claim form before the date OR they just push back the date. Software vendors on the other hand..that's the worry  ::)
