Do billing companies only manage the 3rd party payer component of the billing ... or do they also manage the patient-owed portion, as well?

It depends on the service and what they set up with the provider.  We have many providers that we do both insurance and patient billing for, and we have providers that we only do the insurance portion.  It all depends on the needs of the office.
