Follow up report

Does anyone have a document to show follow up items on that they could send or share with me?  (Besides a report that you print from your system to show A/R follow up) IE a document that lists things the provider asked that needs follow up (filing a certain appeal or exception, looking into a certain code question, any other research etc). To be used for tracking, statuses, etc and that you can show to provider the report so he can see the status on etc?

In my PM software I have a "notes" section and a "pending" status report I use, this report lists all claims or cases in which I am waiting on additional information from the provider. I do it weekly.  Most of it is due to waiting on providers to submit OTR's or even such things as patient insurance changes and need new card.
Good data in your PM system is good data OUT.
