PI Claims

I see some billers handle PI claims (without Medpay).  How would I go about doing that?  My chiro has PI claims that they handle themselves.  Basically they have a lien agreement completed and I guess they keep track of the billings and then when the attorney request their file records, my provider will make copies to send to them and will include their "Invoices" too(which I am not even sure that is the proper way to do it but that is how they do it)...I know these cases can drag on for years (so I am not sure if I would even want to take those on) but how would I add that to my services if I wanted?  Is it profitable and how hard is it to do? What all is involved?

I have billed a few PI claims but we just sent them out on CMS forms.  Payment was a little slow but not years.  We just treated them the same as we would have treated any other commercial insurance carriers.
