Secondary billing basics

I just started doing my own (DME) electronic billing instead of using a clearinghouse and I have a lot to learn.  How do I do a secondary billing?  Do I include primary payment information?

when you are submitting a claim to a secondary insurance, you must also send copy of the primary's eob.

Or if your clearinghouse allows secondary billing you need to check with them to see how to record the primary info.

Recived eob from primary and they made payment and applied a portion to co-ins, deductible or non covered.
Bal billed secondary who we are contracted with, they paid nothing and reduced amount owed by patient to just the co-insurance and deductible.

Sounds like a carve out plan, to which case you can only collect what's in the patient responsibility fields of the EOB.
