Medical neccessity letters

Does anybody know of any books or resources of where I can get some help in creating my own letters of medical neccesity for hospice care?

I would not suggest creating a template for Medical Necessity!  Medical need is different for all patients regardless of the specialty.  You could look for key points that need to be included in every letter and ask your Dr to include those in his letters as to assist you in getting his claim processed with greater ease, but creating generic letters generally is not a good idea.

I fully agree with MNelson.  Every single patient is different in everyway imaginable.
