Psychiatrist Billing/Xoding help needed!
« on: April 03, 2008, 11:28:56 AM »
Hello, I work at a clinic and we are welcoming a psychiatrist to our practice. We really need some direction as to what codes we can bill for his services. We currently have psychologists, but not psychiatrist.  ??? Can anyone help? What codes are typically used and can we use more than one a day?
Thank you for any helo you can give.

Hi,
Do you have a CPT book?  There is a section on psychiatry.  But there are only a few codes that you need in addition to the Psychotherapy codes for psychologists (90801, 90806).  Psychiatrists can use the 90801 and the 90806, but they also use 90862 for medication management (with minimal medical psychotherapy) .  They can also bill E&M codes (99201-99205) if they do a medical evaluation.  There are a few other odd codes they can use for testing and other things.

Good luck, I hope this info is helpful.

Thank you. We are adding that to our clinic and we need to know what the revenue will be. Can I bill medicine management at the same time as a therapy?
I do have the CPT 2008 book, but sometimes people who bill certain services know all of the ways to get the highest reimbursement.
Thank you!

In my previous experience I found that most insurance carriers did not allow you to bill for both the medication management and therapy at the same visit, but it has been a while since I've done that.  Is your psychiatrist going to be doing 45-50 min therapy sessions?  Usually the patient is seen 1 x a month by the psychiatrist strictly for the medication management, but then has therapy throughout the month by psychologist/counselor.  Usually they schedule the therapy at a different visit.

If anyone knows if you can bill therapy using modifier (59) on the same visit as medication management, please let us know.

Thanks
