You don't say what type of provider this is. You will also need to check with the carrier to find out if it's covered and what documentation (if any) you will need. (carriers usually require medical necessity or documentation to establish why the patient could not go to the office) You will also need to use Place of service 12.
For General Medical Home visits (E&M) I believe the CPT range is from 99341-99350.  If the patient's home is a domiciliary, rest home or assisted living facility, you should report codes 99324-99337 rather than home visit codes.
This is from the MGMA's website from a July 2007 FA, so double check the CPT codes.
