Is it possible that we can replace hospital consultation codes 99253-99255 with 99223 when medicare is primary insurance without asking provider????

Are you a coder, or are you the biller?  If you are the biller and the information was given to you with a code of 99253-99255 then you cannot change it without the provider or coder for the provider changing it. 

Codes cannot just be changed to get the claim paid.  The claim is to be coded to the best description of the services performed.  If the services are non covered, then the claim is denied.
