Hi Imran,
You can call the NPI enumerator at 1-800-465-3203 with the info you needed to complete the application. (SS#, address, license, etc.) They will be able to help you.

The NPPES site also has a lookup, and recently added wildcard availability.  We use this almost daily to add the NPI numbers to our referring physician database.  It has become very helpful!  You can search for an individual or organization.

Thanks,  We also use it everyday too.  It's actually easier and much quicker than calling the NPI enumerator.  I would start by trying to look it up on the registry.
