: Flucelvax CPT 90674

Is anyone having any problems with BCBS or United paying or denying this code.  The NDC # I have is 70461030110.

BCBS is stating that the NDC is incorrect and UHC also.  I am having no problems with Medicare paying for it.

Any help will be appreciated.

Are you using the multidose vial? That is not the NDC I find for that. It is 70461-301-10

Yes it is but you have to convert to an 11 digit code that is why there is a 0 after 301

You have to put the 0 at the beginning of the 301 not the end.

70461-0301-10
