billing 15430

I am trying to get paid by medicare for procedure 15430.  my physician is seeing the patient weekly.  we originally used modifier 76, and looks like the first week we get paid, but then the subsequent weeks we get denied global. i saw an article to use modifier 58. 
any thoughts, or advise is more than welcome...

It appears the 15430 has a 90 day global period.  58, 76 or 79 modifer might be appropriate depending on why the 15430 is being done again in the 90 day period.
