We have one insurance company(can't name) that has been denying auto insurance claims do the standard disclosure and acknowledgement form not being filled out correctly. If it's not correct then why are the other companies paying? Do you know anything about the forms Florida requires? Please Help!

I am not familiar with Florida No-Fault specifically but it sounds like they are finding a way out of paying claims.  I would call the company and ask what exactly on the form is not completed properly.  If they can't spell it out exactly what they think is not completed properly, then they should pay the claim.  I know here in NY you can file a complaint with the state insurance commissioner.

Hope that helps you out.  I hate when no fault companies find bogus reasons to deny claims.
