Though it seems a simple question, it's not a simple answer. More info is needed. Is physician participating provider with the insurance carrier being billed? Is this Medicare, Medicaid? Will there be a discount for self-pay and if so will the insurance be billed directly by office or by the patient?

For commercial insurance yes, a person can be self pay.  Often patients with high out of pocket expenses op to be self pay and they usually get a discounted rate.  However this does NOT apply to Medicaid.
