refunds to medicare

is there a specific form that medicare requires if provider office needs to refund a payment?   Here is the situation:  have 2 clients with same name and same dob, however, insurances are different.   the wrong client insurance was filed, medicare has paid, and now I want to refund asap.   any ideas/answers?

If it was filed incorrectly to begin with, you need to take the Medicare remittance you received, file a corrected claim and include the check with a note explaining the error.
