How do you receive information from the providers

I was wondering how you get the information from the medical providers.

Is there a template or do I make one up on my own?

To setup their patients in your system and billing information once they see the patient

It's up to you, you can pickup, which personalizes your services for local providers, you cold use mail, mail carrier or fax. Most use fax as it's just much easier and more efficient. If you are using the provider's software there are also arrangements you can make to where you log in and obtain the necessary information.

We get the information in a different format from almost every provider.  We don't use a template.  Some send over superbills, some send over files, some use a sheet for all patients seen in one day, etc.

We went into an office to see what was going on once.  Dr knew he should have more money coming in but couldn't figure it out.  After running an aging report I quickly saw LOTS of problems, but one in particular was that there were NO inpatient hospital visits on the aging report which told me that there were no inpatient hospital visits being billed.  When we questioned the office manager she said - and I quote (because it's priceless) "I don't bill inpatient visits because he gives them to me on stickies and I don't like that".   

NO JOKE.  He was losing about $250,000 a year! 

Side note ~ she still works for him ~ and that wasn't all we had found.   :o

LMAO.  I wouldn't bill it either.. this one doctor actually when I went to go to his office to pick up contract and first batch of work while I was there, Literally handed me a shoe box full of stickie notes.. I remember him saying "I know this is not the norm, but it's all I have" Now, on the bright side, every stickie note did Have what I needed so basically put them together and then make the provider some daysheets ASAP and he actually said, wow this is much better than the stickie notes and I said .. Yes most anything is going to be better than the stickie notes!~

If a provider does not have her own forms, we offer to create them as part of the start-up package. We create a registration form, verification of benefits form, superbill and encounter form, then teach her how to use each one. This way we know we'll be getting the info that we need. Our clients mail, fax, or scan the forms to us, and I have picked them up in the past as well.

Even then, we still get midwives who will send us an email at 3:00am--"Sarah Smith just had her baby, please bill."  Or, better yet, get a phone call from Sarah Smith--"I had my baby last Friday, can you send the claim?"

We tried to do that with this dr but his wife (also a dr) said "your cute little form doesn't work for me" and "I can't be expected to carry around these forms or a notebook".  As you can see, difficult to work with.

LMAO.  I wouldn't bill it either..

Yes, but at least tell the dr!  She didn't say a word and for over 10 years didn't get a penny for inpatient visits.  Not to mention he hired an NP to be full time hospital for his patients so he paid her salary but didn't get any money!

I LOVE all the new ipad app's..  forms are going to be thing of past soon!!   I have a few clients now using Ipad and all I can say is GOD BLESS MR JOBS AND APPLE!!! :)
