No Fault Insurance claim

Hi Michele, or anyone out there:
Can you advise me on how to file a no fault insurance claim for physical therapy? what procedures do I have to take? what information do I need from the Patient? What forms to use? What code to use?

You need to get the NF carrier information (name of company and mailing address), the policy number, the claim number and the date of accident.  Sometimes you need to have the patient sign an AOB or assignment of benefits form in order for payment to be made to you.  The claims are filed on a CMS 1500 form and the dx & cpt codes are the same as for commercial claims.
