This is not really a good general question. First it's not really a good idea to bill a CPT or ICD-10 based on the "higher reimbursement" services are coded via the physicians documentation. If patient is coming in JUST for a vaccine it would be appropriate only to bill the vaccine. If they are there for a well checkup, focused problem, etc then you would bill the appropriate E/M code with it but again, it's all according to what REALLY was done, not which pays more.
