Speech therapy - Dx's with which CPT ?

patient Dx's are 1) hypotonia 781.3,  2) dysphagia 787.21,  3) coordinaton and functional deficits 315.34, and 4) cornelia de lange syndrome (code ?)

SLP says to bill for 92507 and 92526 per visit, but I need to know which ICD-9 codes go with which CPT codes.  should I use all these Dx? and what is ICD-9 for cornelia de lange syndrome?

Also, when sending documentation to support claim,  provider sent progress notes in an email to me - no signature of rendering provider.  just patient name, DOS, and description of treatment.  She didn't put her name or information on the notes (which I can add) but shouldn't each DOS be signed by her?  My other providers send notes that are signed on each DOS.

You should use the dx's in the order the SLP gave them to you.  SLP should indicate which dx goes with which code.  Also, SLP is responsible to provide acceptable documentation that meet the requirements of the insurance carriers.  I would print out the notes as given to you and attach to the claim.
