: billing for 90966 and 90898

I never billed these codes 90966(End-stage renal disease (ESRD) related services for home dialysis per full month, for patients 20 years of age and older) and 90898(Dialysis training, patient, including helper where applicable, any mode, completed course). I recently started billing for a Internal Medicine provider who works under a Kidney care specialist facility. He provided these services to a patient. I need help for billing with 90966 & 90989. Should I bill them with place of service 65(ESRD treatment facility) and should the month be defined on the claim.  If so in what block should it be denied?
