When billing for a surgeon that saw the patient in the ER, do we put the ER facility in Box 32 with the NPI number and in Box 17? Or does the Dr.s Office information go into Box 32, and Box 17 is blank?

Box 17 is for the refferring Physician information. (Referring Physician Name and NPI#)
Only Box 32 should carry the HOspital name, address, NPI.
