Two Clearinghouses?
« on: April 19, 2011, 05:34:05 PM »
I have a potential new client that origianlly approached me to do only his PI billing (chiro office).  Now he is opening a new location and asked me today about not only doing PI for both practices, but also doing the major medical at his new office.  The problem is that he already has a biller for the main location that he would like to continue to use for the main office major medical.  (Seems like too many cooks in the kitchen to me, but it's his practice.)  When it was PI only, I didn't forsee any problems, but if I am trying to bill MM also and use the same tax id, npi's I think we are going to have a clearinghouse issue.

It is my understanding that if I submit enrollment forms to Zirmed (my clearinghouse) for enrollment with Medicare that it will shut off whatever clearinghouse the other biller is using.  As I think this through, I suppose there wouldn't be any issues with ins cos that don't required enrollment, but for Medicare, Medicaid, etc there would be a conflict.

Does anyone have any experience with a situation like this?

I think you are correct you are going to have trouble with Medicare and Medicaid and any other insurance carrier that requires enrollment to send claims electronically.

I would check with Medicare.  I actually have seen situations with more than one connection.  I would just explain that there are two offices and one will be done differently than the other even though they are the same EIN/NPI.

I took Michelle's advice and called CEDI and was told that I can submit claims from two different submitter numbers, the only problem is that only one of us (me vs the other biller) would be able to receive electronic remits.  I has always thought when you submitted the form to change clearinghouses that it would turn off the other authorization, but in fact it just adds on the new submitter.  Interesting food for thought.  :)
