box 32 - patient treated in the home

 I know, place of service, to use 12 for a patient being treated in his home, but do I need to put the patients home address in box 32, or leave it blank?

Hi,
   Some insurance companies require that Box 32 is completed.  If the services are performed in the home and you've indicated that with the pos 12, then you can leave 32 blank if it is not required, but if it is required to be completed, I would use the office address, or whatever you use for the physical address.
