palmetto gba billing problems

Can you bill cpt 99212 modifier 25 with cpt 17000,17003 dx of 782.19,782.0 and a 11402 with dx of 706.2 with modifier 51 on cpt 11402?Palmetto is stating that this is not paid seperatly.

I'm showing that 782.19 is not a valid dx.  Other than that, based on what you have described it appears that you are billing it correctly.  Which procedure are they denying, the 11402?

They  denied cpt 11402 stating included with another procedure.

Michele:
Was that with the 51 modifier?  If not, then I would definitely put the 51 modifier.  If it had the 51 I'm surprised they denied it.
