Billing Pap Smears

What all can be billed when billing for a pap smear when performed with a well woman exam?  We are billing the Q0091 with a well woman and they are being denied.

They are being denied as a bundled service in with the well woman visit

Many carriers consider the Q0091 as part of the preventative exam.  If that is how the policy is written there isn't anything you can do differently to get it paid.
